Social Work Service

Qualifications:

Medical social services are provided by a qualified Social Worker or by a qualified Social Worker Assistant under the supervision of a qualified Social Worker, and in accordance with the plan of care.

Responsibilities:

Assess social and emotional factors related to the patient's illness, need for adjustment to care and response to treatment.

Assess relationship of the patient's nursing and other medical requirements to the patient's individual home environment, situation, financial resource and available community resources.

Ensure community resources planning and acquisition.

Provide counseling services and acquisition.

Assist in the resolution of problems for high-risk patients.

Collaborate with the physician and other home health team members in understanding emotional and social factors related to each patient's health problem.

Participate in the development of the plan of care.

Prepare clinical and progress notes.

Assist the family in obtaining appropriate community resources.

Participates in discharge planning and in-service programs.

Provide consultation to other agency personnel.
