Registered Nurse

Qualifications:

A graduate of an approved School of Professional Nursing, who is licensed as a Registered Nurse by the State of Illinois, who has no less than on year of medical/surgical experience.

Responsibilities:

Conducts the initial home evaluation and regularly reevaluates the patient's nursing needs.

Initiates the plan of care and necessary revisions.

Performs skilled nursing care an rehabilitative nursing procedures.

Prepares clinical and progress notes.

Coordinates services.

Consults with the patient's physician and other personnel of changes in the patient's condition and needs.

Counsels the patient and family in meeting nursing and related needs.

Participates in in-service programs.

Supervises and teaches other nursing personnel.
