Admission
Agreement
All services are
available without distinction to all program participants
regardless of race, color, national origin, handicap, or age. No
person shall, on the grounds of race, color, creed,
national
origin, or handicap, be excluded from participation in, be
denied the benefits, or the services of, or be otherwise
subjected to discrimination, under any program or activity of
the agency. Services
will be provided to all patients without regard to race,
color, national origin, handicap, or age with the same
requirements that are required by all. The
agency will comply with Title IV of the Civil Rights Act of
1964, the Rehabilitation Act of 1973 and the Age
Discrimination Act and all requirements pursuant to the
regulations. No
patient shall be denied admission to the agency based on
membership in or affiliation with any organization such as a
fraternal or religious group. No
patient shall be denied admission to the agency based on the
physical or mental impairment, medical diagnosis including
patient with contagious diseases including, by not limited to
HIV, MRSA, and hepatitis. Patients
with sensory/hearing impairment shall be provided a qualified
sign language interpreter to meet the needs of the patient at
no cost. The
faculty shall provide a translator for patients who have a
limited English vocabulary to ensure adequate communication to
expedite patient care.
